The Club3D CAC-1088 is an active DisplayPort 1.4 (male) to HDMI 2.1 (female) adapter that lets a PC or graphics card with a DP output drive an HDMI 2.1 TV or monitor at 4K 120 Hz, up to 4K 240 Hz and 8K 60 Hz with DSC. It is a one-way, bus-powered adapter: plug it into the DP port, connect your existing HDMI cable, and no external power or drivers are needed.

HDR passes through, and Club3D notes that HDMI CEC can be enabled via a firmware update, which is useful when the adapter feeds a living-room TV.

Compatibility note: The adapter is uni-directional (DisplayPort source to HDMI display only) — it cannot connect an HDMI source to a DP monitor. 8K 60 Hz and 4K 240 Hz require a DP 1.4 source with DSC 1.2 support and an HDMI 2.1 display; use an Ultra High Speed HDMI cable from the adapter to the screen.

Yes — that is its main use. Connect it to the DisplayPort output of your graphics card and run an Ultra High Speed HDMI cable to the TV; 4K 120 Hz with HDR is supported, and VRR/refresh behaviour depends on the TV and GPU driver.
What is the difference between the CAC-1088 and the CAC-1087?
Both use the same DP 1.4 to HDMI 2.1 conversion. The CAC-1088 is a short adapter with an HDMI female port so you can use any HDMI cable length; the CAC-1087 is a fixed 3 m DP-to-HDMI cable with male connectors on both ends.
